Signed and filled in Form (in case of minors or incapacitated the form should be signed by their legal tutor);
2. 两张护照照片（最近1个月内最新护照照片）passport photograph;

Authentic and valid travel document for more than 3 months beyond the foreseen exit date – photocopy of the passport and previous visas;
4.往返机票预定证明
Reservation of inbound and outbound flight;
5.学生在校证明信/雇主出具证明信 （学校在校证明信须包括:课程名称，课程开始以及结束时间，毕业时间等， 一个月内开具）
Labour-Economic conditions: proof of the employing entity and the salary received in the last 3 months, bank extracts (proof of income) that guarantee the stay and return;
6.旅游保险 （旅游保险额度须涵盖30，000欧元, 便宜网站：Insureandgo）
Travel Medical Insurance (minimum coverage of 30.000 Euros to cover any expenses which might arise in connection with repatriation for medical reasons, urgent medical attention and/or emergency hospital care or death, during the stay(s) on the territory of the Member States);
7.酒店预定证明
Hotel reservation (if applicable) or “term of responsibility”. Guaranteed accommodation by family member or by the host, or a hotel reservation;
8.银行原始帐单，保证旅行时间内 £50 每人每天/或旅行支票 60镑每人每天 （银行帐单出具日期不能超过申请当日一个月）。
Proof of funds in form of the last 3 months bank-statements, the date that issue this letter is no longer than 30 days
9.签证费用 （60欧元签证申请费用，具体价格根据当天汇率有所浮动）
